This presentation will introduce CGRP and its receptors. The molecular composition and pharmacology of these receptors will be explained, together with an overview of their signaling and expression, with an emphasis on cephalic pain.

Calcitonin gene related peptide (CGRP) is widely distributed in the human peripheral and central nervous system including pain pathways. Monoclonal antibodies against CGRP or its receptor have proved to be highly effective for the treatment of migraine. Equally important and in contrast to what could be expected from inhibiting a widely distributed peptide, the drugs are very well tolerated. In fact, the introduction of CGRP antibodies in the field of migraine has been among the largest gamechangers ever seen in the management of migraine. Data from controlled studies on treatment with CGRP antibodies in migraine as well as cluster headache, trigeminal neuralgia, posttraumatic headache, and osteoarthritis will be presented and discussed with the audience.

While the statistical models for machine learning have mostly been developed in the 20th century, with the wide availability of cheap computing and virtually endless data space, data mining and machine learning have become an accessible and mighty tool for researchers outside highly specialized groups. Beyond research, they have become a ubiquitous part of our daily life, as predictive text suggestions on our emails or face recognition to unlock our phones. This presentation will provide an overview of history, methods, and increasing usage of machine learning, from high-end research over daily life improvements to preclinical and clinical opportunities for pain research. A special focus will be set on common misconceptions, inherent bias, and the dangers of (mis)use of computational decision making.
Extracting Useful Epidemiological Data on Pain from Public Datasets

Numerous health-related open datasets exist, many of which span decades. Examples include the Global Burden of Disease dataset, NHS England GP prescribing data, and US health population surveys such as the National Health and Nutrition Examination Survey (NHANES). While these datasets are public, they either were not designed to directly address issues related to pain, or they are heterogenous (within and between surveys) in terms of the pain-related questions that have been asked over time. Nevertheless, these are rich datasets for obtaining high-level epidemiological data on pain, its consequences, and its treatment. In this presentation the wealth of information contained in these datasets and the methods used to identify, extract and analyse such data in order to achieve novel insights into pain at national and global levels will be shown to the audience.

In this workshop we will explore the application of data analytics to such datasets across a diverse range of pain-related topics, demonstrating the flexibility and power of analytical methods to generate novel insights in both basic science and clinical research. Jan Vollert will open the workshop by giving an introduction into data mining and machine learning, setting foundation knowledge about best practices and pitfalls in a fast-evolving, hot topic. Peter Kamerman will speak on using publicly accessible datasets (e.g., global burden of disease data, NHS England prescribing data, and general health survey data from the US) to gain high-level epidemiological insights into pain burden and the management of pain. Finally, Janne Gierthmühlen will present data on predicting development of chronic pain in longitudinal studies of populations at risk, like diabetes.
Moving Beyond the Means and Standard Deviations in Pain Research - Insights to Enhance Personalized Pain Care

Recovery from pain after surgery is usually studied in a cross-sectional manner. This lecture discusses how this method provides information only on the small number of individuals with continuing pain and fails to quantify the burden of pain over time since surgery in everyone. It describes how more frequent assessments of pain after surgery can be acquired and analyzed using growth curve models appropriate for person-centered forecasting and latent class analysis. The increased precision and statistical power of this approach compared to classic repeated measure ANOVA methods when examining interventions to speed recovery after surgery will also be discussed. Data from two recent interventional clinical trials which used daily assessments for 2 months after surgery to define person-centered modeled trajectory as the primary outcome measure will be contrasted with the traditional dichotomous, cross sectional outcome. Finally, challenges to gathering daily data in this research context and approaches to overcome those challenges will be presented.

Using Trajectories of Acute Pain After Surgery to Guide Interventions in a Transitional Pain Service

The Canadian Pain Task Force (CPTF) established the prevention and management of chronic pain as a top priority for Canadians. The Transitional Pain Service (TPS) at the Toronto General Hospital was highlighted as an innovation in pain care. The service has developed over time to secure sustainable multidisciplinary interventions for the prevention and management of chronic postsurgical pain (CPSP) and persistent opioid use following surgery. A review of the known risk factors associated with the development CPSP across the life span will be discussed. Moving beyond risk factors, publications have outlined acute pain trajectories that suggest patterns of pain with a link to the development of CPSP. Within the TPS, pain intensity and opioid use in the days following surgery can guide interventions and signal patients that need increased resources upon presentation to the clinic.

Dr. Brummett will discuss how prescribing after surgery has directly impacted the opioid epidemic in the US and Canada and how the same factors that increased prescribing and morbidity is currently spreading around the world due to changes in opioid prescribing and postoperative care patterns. He will show that while increased opioid prescribing after surgery does not improve pain or satisfaction, it is associated with increased new chronic use, morbidity and cost of care.
